29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) On or about June 28, 2019, employees were working in a trench approximately seven (7) feet nine (9) inches in depth, and approximately fifty-one (51) feet in length without adequate safe means of egress. An extension ladder placed at the west end of the trench was the only means of egress implemented, and required greater than 25 feet of travel for employees working at the east end of the trench.
