On or about June 17, 2019, at the jobsite on coating line two, the Lockout / Tagout program provided to employees operating the Nordson Extrusion Dies Industries (EDI) Ultracoat II Slot Die and the TEKNEK XWCH3 Wide Web Cleaner did not provide the employees a step-by-step procedure or direction on when a LOTO device is required, the purpose of the steps being performed, or how the procedure is protecting employees.
