29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Plant 2, Mixer Machine (old): On or about May 21, 2019 and at times prior to, the employer exposed employees to hazardous energy in the employees were allowed to perform service and maintenance o the Mixer Machine without being provided with a machine specific procedure to lock it out to prevent the accidental start-up of the machine.
