29 CFR 1910.146(d)(2): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not identify and evaluate the hazards of permit spaces before employees entered: Naval OLF NEX: On April 23, 2019, employee(s) were repairing a wireless pressurized line leak detector (WPLLD) in a fuel sump. An employee entered the fuel sump without the hazards of the space evaluated. At the time of entry, the turbine sump was energized and releasing fuel, creating flammable vapors in the space. Employees were exposed to fire and/or atmospheric hazards.
