29 C.F.R. � 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about 4/24/2019, at 20351 Sheridan Street, Pembroke Pines, Florida, the employer failed to instruct employees in the recognition and avoidance of hazards relating to the installation of catch basins, drainage pipes and identification of signs for potential cave-ins for excavations, thus exposing employees to engulfment and cave-in hazards.
