On or about 04/23/2019, Great Clips AdLib Inc., located at 3185 Phoenix Center Drive, Washington, MO, reported a fatality of one their employees. The employee had passed on or about 04/19/2019. Great Clips AdLib Inc. did report the passing of their employee until 04/23/2019, not within the required 8 hour timeframe.
