29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: (a) This violation occurred on or about April 18, 2019, at a workplace located at 2222 Lonnecker Drive, Garland, TX in the Hot Melt department, where employees using an air hose to clean the Kraton Grinder (grater) was exposed employees to an amputation hazard.
