29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Employees were exposed to falls from heights of 4 feet or more as follows: a) On or about March 30, 2019, an employee was elevated on a mobile work platform on a powered industrial truck (order picker). The employee stood on the elevated platform and adjacent pallet in the rack at a height of approximately 5' 10" above the work floor. Personal fall arrest system equipment was available, but not worn. No other means of fall protection was utilized. b) On or about April 8, 2019, an employee was elevated on a mobile work platform on a powered industrial truck (order picker). The employee stood on the elevated platform while wearing a body harness at a height of approximately 5' 10" above the work floor. His harness was not attached to the self-retracting lanyard connected to the anchorage point on the vehicle. No other means of fall protection was utilized. c) On or about April 8, 2019, an employee was elevated on the mobile work platform on a Raymond turret truck at a height of approximately 24' above the work floor and was not protected from falling. Guardrails were not adequate. Personal fall arrest equipment was available, but not utilized. No other means of fall protection was utilized. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
