29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Richardson Tree & Landscaping Co., Inc., fleet yard. On or about April 3, 2019, an employee was struck by a semi-tractor as it was backing up. The tractor was not turned off, and the key remained in the ignition, while the employee was fixing the rear lights of the semi-tractor. No preparation for shutdown was conducted, the semi-tractor was not shutdown, the ignition key was not removed and the vehicle was left running, and isolation and deenergizartion was not verified prior to starting work. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
