29 CFR 1926.502(d)(16)(iv): Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m): a) Jobsite, 12311 Perry Highway, Wexford, PA 15090: On or before March 26, 2019, an employee sustained a fall from greater than six feet from a concrete form. The employee struck the ground and water as a result.
