29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) JMB Printing Press #5: On or about March 14, 2019, the employer did not insure printing press machine specific procedures were utilized by employee(s) when cleaning the surface of stationary components of the system and the ink roller, in-that the roller was not deenergized and was rotating. This exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury. In the alternative... 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect the operator and other employee(s) in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) JMB Printing Press #5: On or about March 14, 2019, the employer did not insure Barrier guarding, interlock and/or light curtains were properly implemented as part of the integrated safety system for each of the four (4) separate color stations. This allowed the employee(s) to access the ink roller(s) in an energized state, and exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury.
