29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: A.) U.S. Bronze Foundry and Machine, Inc., 18649 Brake Shoe Road, Meadville, PA 16335, on or about February 18, 2019 and times prior thereto: Employees were performing service and maintenance work on the Bullard 66" Cutmaster mill (machine #1868). The authorized employees did not affix an energy isolating device to the electrical disconnect.
