29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE): a) EMCOR Pentagon: On or about February 05, 2019, the employer failed to conduct a hazard assessment prior to the bearing replacement for the air handler BF949 motor. Employee had to use 2x4, 4x4 and a 6x2 wooden lever to lift an approximate 360-pound motor. In doing so, the left tip of his middle finger was severed after being caught between the foundation and motor. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
