Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about January 24, 2019, at the chemical feeder port a corrosive chemical was not restrained or rendered safe from spraying chemicals back into the eyes and face of the employee. The employee had just installed a tank float valve on the fresh water feed tank and upon getting information off the side of the boiler a corrosive chemical started to spray out of the port. The employee was exposed to the hazard of burns to the face and eyes from an unexpected release of corrosive chemicals.
