Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) This violation was discovered on or about January 23, 2019; where at least two (2) employee(s) were exposed to tip over of powered industrial truck accidents because of modified front end attachment without manufacturers approval.
