29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 15, 2019, in the combustor cell area, located at 3373 NW 107th Street, Miami, Florida 33167, the employer had not developed and documented LOTO procedures for the 42" Bullard, vertical turret lathe while the employee cleaned turnings from the turntable, exposing the employee to lacerations and broken bones.
