29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Pelletizer #003; On or about January 10, 2019 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work on the pelletizer(s) to clean and clear jams on the cutter without shutting it down and unplugging it in accordance with the company's Energy Control program established procedures.
