29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Facility: The employer did not ensure through providing training and performing evaluation that employees required to operate powered industrial trucks (PIT) were competent to operate the PITs safely at all times. On or about January 11, 2019, a Yale powered industrial truck (PIT), SN A810N04039V, tipped over to a side while the operator was making a sharp turn, and the operator sustained injuries from the incident.
