29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: a) 3475 Piedmont Rd, Atlanta (Construction site) - On or about 1/7/2019, the employer exposed employees to electrical shock and arch flash hazards in that the underground energized electrical power lines were not protected from contact while employees were inside an open excavation cutting and removing three underground power lines to install a new 8 inch ductile iron pipe water supply line inside the trench.
