29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about December 27, 2018, and at times prior thereto, the employer failed to ensure a competent person made frequent and/or regular inspections of the worksite to ensure all workers were in compliance with the company safety and health policies and OSHA standards for proper use of a personal fall arrest system. Workers were exposed to fall hazards when not utilizing a secure point of attachment for their lifeline directly above their work location and the personal fall arrest system was not adjusted to prevent a worker from hitting the ground in the case of a fall. (b) Job Site: On or about December 27, 2018, and at times prior thereto, the employer failed to ensure a competent person made frequent and/or regular inspections of the worksite to ensure all workers were in compliance with the company safety and health policies and OSHA standards for installation and use of personal fall arrest systems. Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2kN) per employee attached, when two employees shared the same anchorage point.
