29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as servicing and/ or maintenance of equipment. a) Sodexo Incorporated, on or about 12/15/18: An energy control procedure was not developed, documented or utilized for the control of hazardous energy while performing servicing activities covered under the lockout standard. On 12/15/18 an employee was clearing a jam from a Little Wonder leaf vacuum and did not ensure that the engine was turned off and locked out prior to reaching inside the machine. As a result, the employee suffered multiple fractures and lacerations to their right arm and hand, including a surgical finger amputation.
