29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Braner Single Loop Slitting Line - On or about December 4, 2018 the employees were exposed to amputation hazards, in that employees were allowed to perform jam clearing operations on equipment such as, but not limited to, Braner Single Loop Slitting Line, without utilizing energy control procedures to prevent accidental startup of the machine.
