29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performed servicing when energization, startup, or the release of hazardous energy could occur, that all energy sources were identified and isolated, and the machinery, equipment, or system was rendered inoperative: USS Miguel Keith, STBD Casing 1-34-1: On November 28, 2018, employees were performing a continuity check on a panel containing energized components. Prior to starting the work, all energy sources were not identified and isolated. An employee contacted two different phases of the line side concurrently with a set of alligator clips, causing an arc flash. The employee was exposed to electrical shock and/or burn hazards from 480 volts.
