29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. On or before November 29, 2018, the employer's hazardous energy control procedures and energy source identification form for the Comco KB-70 bender (model: KB-70NS; S/N: K-02707-N) did not identify the hydraulic energy source and the specific method, device or system to de-energize the power sources to protect employees while servicing and/or performing maintenance activities. b. On or before December 18, 2018, the employer's hazardous energy control procedures and energy source identification form for the Chiyoda SP-90 bender (model: SP-90S; S/N: 8507) did not identify the hydraulic energy source and the specific method, device or system to de-energize the power sources to protect employees while servicing and/or performing maintenance activities.
