29 CFR 1910.184(e)(2)(i): Hooks, rings, oblong links, pear shaped links, welded or mechanical coupling links or other attachments shall have a rated capacity at least equal to that of the alloy steel chain with which they are used or the sling shall not be used in excess of the rated capacity of the weakest component. a. On or about November 14, 2018, an employee suffered crushing injuries to their left index and middle fingers when the company's in-house manufactured tank head hook failed resulting in the surgical amputation of the tip of the employees index finger. The in-house manufactured head hook had not been rated.
