This was most recently documented on November 6, 2018 at the jobsite located at 5699 Indian Trace Road, in Osage Beach, Missouri when the employer did not establish and implement an accident prevention program where employees were exposed to overhead electrical and fall hazards as well as struck-by hazards from equipment.
