29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control. On or about October 25, 2018, in the VFD Building at the Oryx Lease/Pyote Truck Station off Hwy 2355, employees were exposed to electrical arc flash hazards as the employer failed to verify the effectiveness of lock out devices and did not ensure that all components had been tested and locked out with energy control measures before allowing employees to work on reconnecting electrical cables.
