Dowa THT America, Inc. located in Bowling Green, OH: In July 2018, the employer did not develop and implement a written respiratory protection program which covered an employee required to wear tight fitting half face respirator when entering and cleaning out dipping and coating tanks on the LS line, which contained sodium hydroxide and sodium bisulfide residues.
