29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about October 17, 2018, in the saw mill a/c room, below the sawmill dust belt conveyor, the employer did not ensure Lockout/tag out procedures were utilized when an employee performing troubleshooting on jammed dust belt conveyor was exposed to a caught in hazard that resulted in an amputation.
