29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system. a) On or about October 19, 2018, or prior thereto: The Onion Bin Tipping Machine had a 12 inch wide worker platform with an inadequate guardrail system. The toprail was 31 inches above the platform and did not comply with requirements for guardrail systems in 29 CFR 1910.29(b)(1) and (b)(2). The platform was located above an onion hopper with a conveyor system. b) On or about October 19, 2018, or prior thereto: The Onion Bin Tipping Machine had a 12 inch wide worker platform with an inadequate guardrail system. The platform was not guarded by a mid-rail, and did not comply with requirements for guardrail systems in 29 CFR 1910.29(b)(1) and (b)(2). The platform was located above an onion hopper with a conveyor system. c) On or about October 19, 2018, or prior thereto: The Onion Bin Tipping Machine had a 12 inch wide worker platform with an inadequate guardrail system. The top rail was 31 inches above the platform and was offset on an angle to the walking/working surface. The top rail was not perpendicular to the platform. The platform was located above an onion hopper with a conveyor system. NOTE: Abatement certification is required for this item.
