29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about September 27, 2018, the employer did not ensure that employees who operated the Doboy wrapper machine (s/n 80-11361) located at line 9 were protected from injuries relating to rotating part hazards. Employees were exposed to amputation injury in that the in-going nip point, created by the rotating crimping wheels, were not guarded. Hearthside Food Solutions was previously cited for an equivalent Occupational Safety and Health Standard 1910.212(a)(1) which was contained in OSHA inspection number 1081991, citation number 2, item number 1and was affirmed as a final order on January 19, 2017, with respect to a workplace located at 312 Rader Rd., McComb, Ohio 45858.
