29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) Jobsite: On or about September 13, 2018 and at times prior thereto an eight foot trench box was not high enough to provide the necessary protection for employees working within the trench box. Soil from an excavated side wall cracked and came over the top and into the trench box. The trench was between 9 and a half to 10 and a half feet deep with type C soil. b) Jobsite: On or about September 13, 2018 and at times prior thereto employees working within an eight foot trench box were not protected on the end from the backfill material and soil from a collapsing side wall. A chunk of soil rolled into the end of the trench box, striking an employee. The trench was between 9 and a half to 10 and a half feet deep with type C soil.
