29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: On or about August 22, 2018, the employer failed to ensure that lockout procedures for the Baker lines 5 head Keystone cutoff saw, sizer, 2 head Keystone band saw, and the Defrazzler covered a specific statement of the intended use of the procedure and the specific requirements for testing the machines to determine and verify the effectiveness of lock out devices and other energy control measures. The employees were exposed to electrical and hydraulic powered rotating and moving machine parts when changing blades, adjusting belts, and clearing jams.
