29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Shaving machine #3, on or about August 20, 2018, or prior to, an employee entered into a shaver box that contained rotating shaving heads, without utilizing the lockout procedures and shutting the shaving heads down and locking the machine out, exposing the employee to an amputation hazard.
