29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) HVAC Duct Floor Openings on 2nd, 3rd and 4th Floors - On or about August 15, 2018 and at times prior to, the employer did not ensure each HVAC duct floor opening was guarded, thus exposing employees to fall hazards of up to approximately 30 feet.
