29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect employees from amputation and struck-by hazards. This was most recently documented at 1351 S. Reca Ct, Wichita, Kansas, where employees were removing and replacing the Boom Hoist Cylinders on a Lull Highlander. The employees were exposed in that an equipment specific energy control procedure had not been developed, documented, and utilized while engaging in the following activities: a) On or about 07/27/18, while replacing the right Boom Hoist Cylinder on the Lull, Model 1044C-54. b) On or about 08/03/18, while replacing the left Boom Hoist Cylinder on the Lull, Model 1044C-54
