Text For Citation: 01 Item/Group: 001A Hazard:
OSH ACT of 1970 Section (5)(a)(l): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in-between and crush hazards: (a) Facility, 71 Progress Avenue, Cranberry Township, PA: On or about July 20, 2018 the energy control procedure for the tilt table did not specifically outline a step for isolating, blocking or securing the operating switch for the tilt table to ensure there was no operation or release of hazardous energy when engaged in activities covered by this section. (b) Facility, Assembly area, 71 Progress Avenue, Cranberry Township, PA: On or before July 18, 2018 an energy control device was not available or placed on the switch on the tilt table. The airline had been removed by from the table by an employee who was to clean in and around the machine and bander. Unbeknown to the employee working at the table another employee turned the switch for the table on and when it didn't function, walked away. The employee who was cleaning, finished and reattached the airline to the table. The table immediately began to lift and trapped the employee between the table and the floor. The switch to the table had not been locked or tagged to isolate the machine and control the energy. (c) Facility, Assembly Area, 71 Progress Avenue, Cranberry Township, PA: On or about July 20, 2018 retraining had not been provided for authorized and/or affected employees who had a change in their work assignment that included servicing equipment to ensure they understood the process and procedures of the control of energy for equipment used. Among other feasible and acceptable abatement methods to correct this hazard is to ensure that: all steps in the employers lockout tagout program are clearly outlined, all necessary equipment to isolate all energy sources are provided and that supervisors follow the employer's written lockout tagout program.
