29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: (a) Turn Key General Contractors, Inc. at 6305 E Hampden Ave #102, Denver, CO 80222: On July 16, 2018 the employer did not ensure all wheels were secured on the mobile scaffold in use, in that a wheel rolled into a hole in the cement floor. This condition caused the wheeled scaffold to tip over, resulting in employee injury. The wheels exposed employee to struck-by and fall hazards when the mobile scaffolding he was standing on tipped over.
