29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: a) Courtyard Area- On or about July 5, 2018, the employer did not ensure that employees utilized a lockout procedure that provided employees a level of protection equivalent to that provided by the implementation of a personal lockout device to protect them from the unexpected energization of the equipment when work was performed by a group on the Glue and Roll Coat Lines Oxidizer 1097. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
