29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 07/10/18 at the Castile location; employees were exposed to amputation hazards when changing the blades on the rail end shaper # 12 in the Shaping and Boring department when the employer did not enforce the use of the written LOTO procedures during these activities which included attaching a lock to the local disconnect. NO ABATEMENT CERTIFICATION REQUIRED
