29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: On or about June 19, 2018, and at times prior there to, employees were performing lifting activities without properly balancing the load in a chain sling. The employees were exposed to a struck-by hazard.
