29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) Underground concrete vaults and steel gasoline tanks numbers 1, 2, & 3 - On or about June 12, 2018, the employer did not properly evaluate underground tank vaults to determine if they were permit-required confined spaces, and ensuring acceptable entry conditions were met before employees were permitted to conduct entry and attendant activities.
