29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that would have prevented employee exposure to hazards: (a)First Dakota Enterprises, Inc. at 3500 Grand Slam Avenue, Sioux Falls, SD 57104: On and about and at times prior to May 31, 2018, the exposing employer did not ensure employees were protected from cave-in hazards in that, employees performed work within a GME trench box without all the pins installed in the spreaders to collar connections . This condition exposed employees to cave-in hazards and struck-by hazards.
