29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system. On or about 5/15/2018 at 1298 Old Alpharetta Highway Alpharetta, GA, workers were exposed to fall hazards of 11-feet, 4-inches to the concrete lower level from the mezzanine during activities including but not limited to removing chains in preparation for movement of material by forklift.
