29 CFR 1926.950(b)(2)(iii): Each qualified employee was not trained and competent in the minimum approach distances specified in this subpart corresponding to the voltages to which the qualified employee would be exposed and the skills and techniques necessary to maintain those distances: Site- Employees exposed to live overhead conductors operating at 34.5 kv while setting a 50 foot utility pole between live conductors were not provided with training and competent in the minimum approach distance established by the employer.
