29 CFR 1910.67(c)(2)(ii): Only trained persons shall operate an aerial lift: On or about March 30, 2018, the employer failed to provide formal instruction and/or training to employees on how to safely operate a rented articulating boom lift (Genie TZ50) prior to allowing employees to operate the equipment. The untrained workers were tasked with dismantling/removing an old roll up garage door, and were working from the basket of the boom lift at a height over 13 (+) feet. 29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: On or about March 30, 2018, the employer failed to ensure that employees using a 50 foot (Genie TZ50) articulating boom lift during the removal/dismantling of an old roll up garage door were tied off or connected to the boom or basket of the lift by either a personal fall arrest or travel restraint system. The employees were working at a height of 13 (+) feet were exposed to fall hazards.
