29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The employer is failing to protect employees from struck by and crushing hazards associated with servicing and maintaining equipment with stored energy. This was most recently documented on March 22, 2018 at the facility located at 431 A Street, Lincoln, NE 68501 where the employer has developed inadequate written specific procedures for the maintenance of the bridge tags on the fleet of Mack concrete mixer trucks. The procedures do not identify gravity and hydraulic pressure as potential sources of stored energy nor do they provide instruction on methods to secure the bridge tag to prevent it from swinging down and striking an employee.
