29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. a. Toledo Tool & Die Company, Inc. located in Toledo, Ohio: On or about March 23, 2018, the employer did not ensure employees performing die setting on the 150 ton Aida press #81 located in the production area, were trained to lockout the equipment during die setting operations. The employee was exposed to crushing injuries between the upper and lower dies when performing the work. b. Toledo Tool & Die Company, Inc. located in Toledo, Ohio: On or about March 23, 2018, the employer did not ensure an employee performing a steel coil change over on the 250 ton Komatsu press #90 located in the production area, was trained to lockout the equipment during coil changeover operations. The employee was exposed to crushing injuries between the upper and lower dies when performing the work.
