29 CFR 1910.147(c)(2)(ii): When an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section did not utilize lockout: a) Doren, Inc. in Wampum, PA. On or about March 20, 2018, employees dismantling the Tiger Machine did not lockout all of the energy sources that were capable of being locked. Components related to the machine were shut off at the electrical panel and the switches were covered with duct tape.
