29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Alex Products, Inc., located at 8110 Gasser Road, Paulding, OH 45879: On or about March 6, 2018, the employer did not utilize specific procedures to control hazardous energy of the equipment such as, but not limited to, the wire draw machine located at the Draw Line # 2. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before performing activities such as, but not limited to, setting-up the machine. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
